Voluntary Separations
must be completed by either the employee separating, their HEERA Manager, or HEERA Manager's Delegate to initiate a notice of separation and property clearance. This form is applicable only for:
- Full time & Part time Faculty – resigning, retiring, or entering FERP.
- Staff and Management – resigning or retiring.
- Temporary Faculty, Staff and Management - ONLY if permanently resigning from temporary employment contract.
Employees holding more than one employment should specify the particular position they are resigning from to avoid fully terminating their 六色网 employment.
If an employee cannot complete the process, Faculty should contact their Department Chair and Staff/Management should contact their first level manager (MPP) or their manager’s delegate for assistance.
Information
- Submit the in ServiceNow.
- Follow the step-by-step process in the Separation & Clearance Guide (PDF) for additional support.
- Provide a formal Resignation Letter (DOCX) to the appropriate administrator.
- For staff/management, this is the first level manager in the management personnel plan (MPP) to whom the employee reports to.
- For faculty, this is the HEERA Manager/College Dean.
- It is not necessary for a temporary employee to complete a resignation letter unless the appointment is ending prior to the original end date, and they are initiating the separation.
- PRIOR to Last Day Worked.

- Return resources and property in your possession and pay any financial obligations.
- This includes the return of procurement cards, parking decal, library asset, key(s), IT equipment, final reconciliation of all outstanding procurement card statements, and any other financial obligations.
- Review and complete the Separation Checklist for Employees (DOCX).
- Participate in a confidential exit survey.
- EODA/HR Operations will provide an online exit survey shortly after a notice of separation is submitted. An exit interview may also be scheduled with EODA/HR Operations.
- Complete the Documentation of Work and Committees form (DOCX).
- This worksheet is intended as a guide for the employee to document their work and committees they serve on.
It is the manager’s responsibility to ensure all absence management is approved, property is collected, all outstanding reconciliations are complete, and reimbursements owed to the university are collected prior to the employee’s last day of work.
- Ensure the separating employee completes the in ServiceNow. If the employee isn't able to complete this form, the manager or division HR coordinator should complete on their behalf.
- Approve separating employees time entries in absence management.
- Confirm the return of University Property and Equipment, Campus Identification Card, Keys, Purchasing Cards, and the settlement of any outstanding accounts.
- Confirm all purchasing card reconciliations have been completed.
- Confirm all travel expense claims have been completed.
- Confirm Separation Checklist for Managers (DOCX) has been completed.
- Confirm that the departing employee has completed the Documentation of Work and Committees form (DOCX).

In the survey, you will be able to inform Employee/Labor Relations & Compliance if you wish to participate in a confidential exit interview as well. Your honesty is greatly appreciated, and your opinions are highly valued. Your feedback provides insight into the work environment here at CSU Pomona and the factors that may lead to an employee’s decision to leave.
