Employees should maintain current employment and payroll records, including addresses. To change and/or update your address on-line, faculty/staff may utilize Employee Self-Service through the  homepage.

Otherwise, please follow the steps outlined below for Faculty & Staff or Student Assistants.

Please don't forget to update your Emergency Contact (pdf) information if it has changed due to your move/change in address.

Faculty & Staff

Unless using Employee Self-Service on BroncoDirect, address changes are to be filed using the . The EAR document is a State of California form (Std. 686) used to collect an employee's home address, as well as other information.

To file an address change, faculty and staff employees should complete the EAR form, print it, and submit it to the representative at the Customer Service Center, located on the second floor of the SSB 121 W 2700. (Note: We do not recommend that you mail your completed form to the Customer Service Center, due to the confidential information included on the form--i.e., your Social Security Number. Also, since we require an original signature, we do not accept faxes).

Important Notes Regarding Address Changes:

  • EAR Forms cannot be filed through the mail, electronically, or by fax--you must submit the EAR in person to the Customer Service Center and present your Bronco Card for identification purpose.

  • Employees may not use a CSU address as a personal address on the EAR document unless the employee is authorized to and resides in a CSU-owned property, such as a campus dormitory.

If you have other deducations (Credit Union, Automobile Insurance, etc.), you must change your address with the deduction company. This form does not affect an address change with deduction companies or organizations.

Student Assistants

It is recommended that student assistants update their address for employment and payroll purposes by changing the "home address" field on .

There are multiple address fields on My六色网; however, the home address field will automatically update your address in our system.

It is important to update your personal information when changes occur. Should your name change, you will need to update your record with the University for employment and payroll purposes.

Update Your Social Security Card First

To file a name change, you will need to first update your record with the Social Security Administration and obtain a new Social Security Card that reflects your new name. Federal and State regulations, as well as CSU policy, require that we validate that each employee's name and social security number on record exactly matches the name and social security number that appears on the individual's Social Security Card to ensure proper tax reporting.

Filing Your Name Change with Cal Poly Pomona

Once you have a Social Security Card which reflects your new name, you will need to complete the  if you are faculty/staff, or the if you are a student assistant. A Customer Service Center Representative , available by telephone at extension 3733, can help you identify which form to use.

To file your name change, please take your completed form to the Customer Service Center, located on the second floor of the SSB 121 W 2700. You will need to bring your Bronco Card for identification and your new Social Security Card for verification purposes.

Important Notice: EAR and SPAR Forms cannot be filed through the mail, electronically, or by fax--you must submit the EAR or SPAR in person to the Customer Service Center and present your Bronco Card for identification purposes and your new Social Security Card so we may verify that it matches your name as indicated on your name change request.

This designation is for the issuance of pay warrants only after an employee death. It has no impact on your beneficiary information for your retirement benefits or life insurance. You may change your designee at any time by completing a new .

The form is available through Payroll Services' website. However, it must be signed and submitted in person at the Customer Service Center, Students Services Building West 121-2700. The designation does not become effective until the form is signed and submitted at the Customer Service Center.

A withholding allowance certificate ( W-4) must be completed by every CSU employee, to designate your individual federal and state tax withholding information, such as:

  • Whether to withhold at the single rate or at the lower married rate.

  • How many withholding allowances you claim (each allowance reduces the amount withheld).

  • Whether you want an additional amount withheld.

Employees will complete one of the following two forms used, in lieu of Federal Form W-4, as withholding allowance certificates within the Uniform State Payroll System (USPS) by the State Controller's Office:

  • Faculty/Staff complete the 

  • Student Assistants (class codes: 1870, 1871, 1872) complete the

Note: You must specify a filing status and a number of withholding allowances. You cannot specify only a dollar amount of withholding.

Changing Your Withholding

Events during the year may change your marital status or the exemptions, adjustments, deductions, or credits you expect to claim on your tax return. When this happens, you may need to file a new EAR (Faculty/Staff ) or SPAR (Student Assistants) to change your withholding status or number of allowances.
